I hereby assume all of the risks of participating in any/all activities associated with performing the programming I am given via Performance Coaching provided by Complete Human Performance and their coaches, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.


I certify that I am physically fit, have sufficiently prepared or trained for participation in this activity, and have not been advised not to participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity.


I acknowledge that this Release of Liability and Accident Waiver Form will be used by Complete Human Performance coaches, and that it will govern my actions and responsibilities during associated activities.
In consideration of my application and permitting me to participate in this activity, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors and assigns as
follows:


A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including traveling to and from this activity. THE FOLLOWING ENTITIES OR PERSONS: Complete Human Performance and/or their directors, officers, employees, volunteers, representatives and agents, and the activity holders, sponsors, and volunteers;
B)  INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the persons or entities mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise. 


I acknowledge that they are NOT responsible for the errors, omissions, acts, or failures of any party or entity conducting a specific activity on their behalf. I acknowledge that this activity may involve a test of a person's physical and mental limits and carries with it the potential for death, serious injury, and property loss. The risks include, but are not limited to, those cause by terrain, facilities, temperature, weather, condition of participants, equipment, vehicular traffic, lack of hydration, and actions of other people including, but not limited to, participants, volunteers, monitors and/or producers of the activity. I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity.

The Accident Waiver and Liability Release Form shall be constructed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

If I choose to follow the Special Segments and Shooting Drills, I guarantee that I will do so lawfully, safely, and carefully, following the direction given within the template and the laws of my state or country.

Extreme Performance Protocol Template

The closest thing to a Military Selection template we've ever made. 

What you'll get:

  • Includes 2 protocols, "Prepare" and "Perform," with additional Special Segments (Swim, Ruck, Run, Movement and Navigation, Agility) and Shooting Drills
  • "Prepare": This program is the one that the majority of trainees, even those with specific performance goals related to their career, will want to start with.  This program is aimed at making you as brutally strong, fast, and durable as possible.  It is an aggressive training program, combining heavy lifting with both speed work and longer duration conditioning.  Agility and plyometrics are not neglected, and many days will have multiple “sessions”, which can of course all be combined into a single session.  This program assumes greater training time/availability than the second program, and can be run repeatedly with minimal adjustments.
  • "Perform":  This program is focused on active duty military/LE or tactical sport athletes who are looking to improve not just their raw physical parameters, but leverage their physical fitness for combat/live fire related careers or sports. It combines resistance training with endurance training, while also incorporating functional medleys and metabolic conditioning and shooting/combat drills. Multiple options allow it to be tailored for sidearm/pistol, rifle, or bow shooting, and conditioning may incorporate swimming, rucking, backcountry skiing, or any other discipline needed by the individual. This program may also be run repeatedly, though we never object to rotations.
